Rockvale Middle School Medication Information

 

All Medication given at Rockvale Middle School will be provided by the parents or legal guardians. This medication will be self-administered by the nurse or personnel trained to assist with medication that are self –administered to students. Please do not bring medication to school that will make your child sleepy or disoriented. If this medication is needed please keep him / her at home until the medication is not needed. If your child will need medication during the school year please read the directions below for the correct way to bring medication to school for your child. AUTHORIZATION TO ASSIST COMPETENT STUDENT WITH SELF-ADMINISTRATION OF MEDICATION

 

Medication shall be dispensed only when the student’s health requires that it be taken during school hours.  The medication must be delivered to the nurse’s office in person by the parent/guardian of the student or the parent’s adult designee unless the medication must be retained by the student for immediate self-administration (i.e. students with asthma).  It is the parent/guardian’s responsibility to remove any unused medication when treatment is completed.

 

All prescription medication must be brought to school in the original container.  The pharmacy label must include the following information:

·     Name of student

·     Prescription number

·     Name of medication and dosage

·     Administration route or other directions

·     Date

·     Licensed prescriber’s name

·     Pharmacy name, address, and phone number

 

IF THE MEDICATION IS FOR A CHRONIC OR LONG TERM MEDICAL CONDITION, (OVER THREE WEEKS), OR IF REQUESTED BY THE SCHOOL NURSE, A PHYSICIAN’S / LICENESED HEALTH CARE PROVIDER’S SIGNATURE IS REQUIRED.

 

All non-prescription medication must be brought to school in the original manufacturer’s labeled container with the ingredients listed and the child’s name affixed to the container.

 

It is recommended that no more than one month’s supply of any medication should be brought to school.

The parent or guardian is responsible for informing the designated official of any change in the student’s health or change in medication.
